B'NAI HAVURAH IS AN  INCLUSIVE COMMUNITY

It is our policy that B’nai Havurah members are included and welcomed to all B’nai Mitzvah services as well as the Kiddush and Motzi (blessing for wine/juice and challah) following services.

We will help you estimate the number of people to plan for at the Kiddush following services. It is up to you whether or not to include B’nai Havurah members in the meal following services, though we request that My Mitzvah classmates (not necessarily their parents) of your student(s) are invited to the meal following services.

Normally, we see a maximum of six B'nai Havurah members who opt to stay for the meal following the ceremony.

FOR ONSITE B'NAI MITZVAH
Please answer the following questions if your B’nai Mitzvah is onsite

Please skip to the next section of this form, "For Offsite B'nai Mitavah", if your event is not at B'nai Havurah

We can make arrangements for pick up and return of the kippot along with other plans for pick up the Torah, siddurim, and ark. 
We require proof that your selected caterer is licensed and insured.
Leave blank if you have not had a site visit with Becky or Adrianna yet and do not know. Also, feel free to have your caterer discuss this matter with Becky or Adrianna.
A photographer or videographer, for instance, may want early access to the building

FOR OFFSITE B'NAI MITZVAH
Please answer the following questions if your B’nai Mitzvah is offsite

This assumes in either case (B'nai provides the challah, wine/grape juice or you/your caterer does),  we will provide two ounce clear plastic cups and a challah tray and covering, unless you tell us otherwise.
We request that if you plan to use the B’nai siddurim that someone is responsible for picking them up and returning them to B’nai Havurah. It is this persons responsibility to contact the office at least two weeks before the B’nai Mitzvah to make arrangements for the pick up and drop off the siddurim.
Please contact our office at least three weeks prior to your B'nai Mitzvah to confirm plans for the pick up and return of our Torah scroll. Our Torahs are priceless and require careful handling. 
If you have not provided us with with the name and contact information of the individual to transport a Torah scroll to and from B'nai Havurah, our Rabbi or Mitzvah Mavens will transport the Torah.
The person picking up the ark is asked to contact B'nai Havurah at least three weeks prior to the ceremony to make arrangements.  A large SUV or van is required to accommodate the ark.  Under no circumstance may the ark be tied to the top of a car or left outside overnight. 
It is this persons responsibility to familiarize themselves with the instructions for setting up the ark.  We provide written instructions and encourage those who not familiar with the ark to mention this when they contact our office to arrange for pick up.

B’nai Mitzvah Certificate Information

If a parent does not have a Hebrew name, we will transcribe the English into Hebrew letters.
